Healthcare Passport

If you don’t have one already, you may find it useful to you. You can fill this in and we will add it to your records so when you come to see us, or perhaps have to go to the hospital, they will already know lots of things about you to help you if you are feeling poorly. Reasonable Adjustments

Reasonable adjustments are small changes that can help people with a learning disability be treated equally. It is your right to ask for reasonable adjustments when you have an appointment for your health.
